Poor Response to FIP Treatment

Updated: Aug 18, 2023

Typically, the dosage of the GS441524 antiviral drug remains unchanged throughout the treatment course, but the dose should be recalculated and adjusted on a daily or weekly basis in response to any changes in the cat's weight. Dosage of GS441524 antiviral drug wet FIP = 6mg/kg SID SQ

dry FIP = 8mg/kg SID SQ ocular FIP = 10mg/kg SID SQ neurological FIP = 10mg/kg SID SQ

However, if the FIP cat is not showing improvement, it may be necessary to increase the dosage of the GS441524 antiviral drug. It is not recommended to decrease the dosage as this can increase the likelihood of developing resistance to the antiviral drug, except in cases where the cat was initially started on higher dosing and then tapered down within the first 3-5 days.

Dosage adjustments may be needed in the following situations:

  • Manifestation of neurological or ocular symptoms

  • Lack of clinical improvement

  • Lack of improvement in bloodwork

  • Relapse of symptoms

If effusions are not resolving within the expected timeline, it is important to first rule out other potential causes before considering a dosage increase.

Before raising the dose, it is recommended to confirm that the cat owner is administering the correct dose and that there are no issues with the cat's compliance with taking the medication. Problems with administration and compliance are common causes of treatment failure.

When increasing the GS441524 antiviral dosage, it should be done in specific increments of +2mg/kg for slow treatment progress or +4 mg/kg for relapse cases.

If an increased dosage elicits a positive response, it should be continued for a minimum of 4 weeks. This may require extending the FIP treatment beyond the usual 12-week duration. In the case of a relapse or significant regression during treatment, a minimum of 8 weeks from the point of dosage adjustment may be necessary. This could result in an extended treatment period beyond the usual 12 weeks.
